Doncaster Mind – HIU community coach

Doncaster Mind has a unique service that supports people with a variety of health and wellbeing needs who are regularly accessing emergency services. If you feel that you can make a difference in someone's life, we're recruiting for a full-time HIU community coach to join our dedicated team.

What you'll do

In this role you'll:

• Offer effective one-to-one, coaching-style support both face-to-face and via telephone
• Put in place interventions that reintegrate people into the local community
• Develop and maintain excellent links and relationships with other agencies and community services
• Monitor client progress
• Maintain accurate client records and contact notes

About you

We're looking for someone with:

• Creative, compassionate and patient attitude
• Appetite for helping those that others may have either forgotten about or turned away
