York Mind – social prescriber

York Mind is looking for a social prescriber to help people improve wellbeing and access community support. 

What you'll do

In this role you'll:

• Support a caseload of people to access community based interventions that improve health and wellbeing 
• Co-produce personalised action plans to build independence, resilience and self management.
• Work closely with multi agency partners across voluntary, statutory and health services.

About you

We're looking for someone who:

• Has experience supporting people with diverse and complex mental health or social care needs.
• Is skilled in building trusting relationships using active listening and motivational approaches.
• Can manage a varied caseload, work independently, and prioritise effectively in community settings.
