York Mind seek a social prescriber to connect people with community support and improve wellbeing.
What you'll do
In this role you'll:
- Support people to improve their health and wellbeing through personalised social prescribing and community connections.
- Work with individuals to create action plans that build independence, resilience and safe management skills.
- Develop strong relationships with local services and organisations to help people access the right support.
About you
We're looking for someone who:
- Has experience working with people with complex mental health and wellbeing needs.
- Can build trusting relationships and motivate people to make positive, lasting changes.
- Has experience of multi agency working, safeguarding and supporting people to access community resources.
