Solent Mind - Wellbeing Assessor (PCN) - New Forest

Join Solent Mind as an Wellbeing Assessor in the New Forest to support people with their mental health and wellbeing.

What you'll do

In this role you'll:

• Carry out wellbeing assessments for people experiencing mild to moderate mental health challenges, identifying their needs and appropriate support options

• Provide one-to-one wellbeing support through face-to-face, phone and online sessions, helping people set goals and build emotional coping skills

• Work closely with GPs, mental health practitioners and community partners to connect people with relevant services and ongoing support

About you

We're looking for someone who:

• Has experience supporting vulnerable people with diverse needs, ideally within a mental health or similar setting

• Understands mental health challenges and can assess individual needs to identify appropriate wellbeing support

• Has strong communication, IT and teamwork skills, with the ability to work effectively in a multidisciplinary environment
