Oxfordshire Mind – wellbeing worker

We're looking for someone who is motivated, proactive and supportive, with a strong commitment to person-centred work and service user involvement.

You'll be part of a fast growing team of wellbeing workers from all walks of life, providing a mix of face to face and telephone work in the GP practice team.

What you'll do

You will:

• Support service users to improve or maintain their mental health and wellbeing

• Collect and accurately record service user data onto a secure database 

• Work with colleagues, partner organisations and professional agencies to support service users

• Keep up to date with the services offered by NHS and third sector providers

• Carry out regular risk assessments and manage the risks related to staff, service users and buildings 

• Respond to referrals from different sources, like GPs, community organisations and self-referrals

• Make sure people receive an excellent person-centred service via an initial assessment and follow-up sessions 

• Promote the service so it's well understood and easily accessible 

• Use support tools and offer tailored signposting to help service users to achieve their goals
