Oxfordshire Mind - community connector 

Oxfordshire Mind are looking for a community connector to link people to mental health and wellbeing support. 

What you'll do

In this role you'll:

• Support people to access community services that improve their health, wellbeing and independence.
• Work alongside NHS teams to assess needs, develop support plans and provide practical interventions.
• Build partnerships and map local resources to strengthen support available across the community. 

About you

We're looking for someone who:

• Has experience working in mental health, wellbeing or similar community based services. 
• Understands safeguarding and can support people with a range of needs in a safe and effective way.
• Is committed to person centred strengths based support and building positive relationships.
