Bradford District and Craven Mind - health engagement practitioner

Bradford District and Craven Mind seek a practitioner to support people with SMI to access health checks.

What you'll do

In this role you'll:

• Support people with severe mental illness to access physical health checks and improve their overall wellbeing.
• Build trusting relationships and use person centred approaches to engage people who may be disengaged from services.
• Work alongside primary care teams and partners to provide joined-up holistic support. 

About you

We're looking for someone who:

• Has an understanding of mental health and the needs of people living with severe mental illness.
• Is confident building relationships and supporting people in an empathetic, empowering way. 
• Enjoys working collaboratively with health professionals and community partners.
