We're really glad that you've decided to send us your mental health story. At Mind we know that real people's experiences help people facing mental health problems.
Before you send us your story, please read these Terms and Conditions carefully. If you agree to them, tick 'yes' and then click 'next'.
*Please note that we can only accept submissions of stories from people over 18.
I give permission for Mind to use my video in the Mind website and other Mind sites including social media (e.g. Youtube, Facebook, Twitter etc.) as part of their work to support people with mental health problems
I am happy that they can share it via email social media (such as Facebook and Twitter) and in emails to their supporters.
I am happy that they may use my mental health story in communications with potential or actual partners and supporters (e.g. internal and external training, possible donors, corporate partnerships etc.)
I agree that I am happy for them to edit parts of my 'mental health story' and that they may go alongside other professional or volunteer's videos
