York Mind seek a social prescriber to connect people with community support and improve wellbeing.
What you'll do
In this role you'll:
- Help people improve their health and wellbeing by connecting them with community activities, services and support that meet their individual needs.
- Work with people to create personalised action plans, helping them build resilience, confidence and independence through strengths based approach.
- Develop partnerships across health, voluntary and community services to improve access to support and reduce inequalities.
About you
We're looking for someone who:
- Has experience supporting people with diverse health social care or mental health needs, including people with complex needs.
- Can build trusting relationships, motivate people to make positive changes and work effectively with a range of partner organisations.
- Bring strong communication, assessment and organisational skills, with an understanding of safeguarding, confidentiality and community support pathways.
