York Mind seek a social prescriber to connect people with community support and improve wellbeing. 

What you'll do

In this role you'll:

• Help people improve their health and wellbeing by connecting them with community activities, services and support that meet their individual needs. 
• Work alongside individuals to create personalised action plans, building confidence, resilience and independence through strengths based approach. 
• Build relationships with local organisations and partners to help people access opportunities that support positive and lasting change. 

About you

We're looking for someone who:

• Has experience supporting people with diverse health, social care or mental health needs. 
• Can build trusting relationships, motivate people to achieve their goals and work effectively with a range of partners. 
• Brings strong communication, assessment and organisational skills, with an understanding of safeguarding and community support pathways.
