Solent Mind seek a wellbeing assessor to support people with diverse mental health and wellbeing needs. 

What you'll do

In this role you'll:

• Support people with diverse mental health and wellbeing needs through person centred assessments and ongoing support. 
• Work collaboratively with colleagues and partners to help people access the right services and achieve their goals.
• Build trusting relationships that empower individuals to improve their wellbeing and resilience. 

About you

We're looking for someone who:

• Has experience working with vulnerable people with a range of needs in a mental health or similar setting.
• Brings strong communication, teamwork and IT skills. 
• Is compassionate, person centred and committed to helping people achieve positive outcomes.
