Bradford District and Craven Mind – health engagement practitioner

Bradford District and Craven Mind seek a health engagement practitioner to support people with severe mental problems. 

What you'll do

In this role you'll:

• Support people living with severe mental illness to access physical health checks and improve their overall wellbeing. 
• Build trusting relationships with people who may be disengaged from services, using positive and person centred approaches.
• Work alongside primary care teams and partners to help people access the support and interventions they need.  

About you

We're looking for someone who:

• Has experience supporting people with complex needs, including mental health challenges. 
• Can engage with people in a compassionate, non judgmental and solution focused way.
• Is confident managing risk, responding to safeguarding concerns and working collaboratively with partners.
