Bradford District and Craven Mind seek a pathways worker to help young people access mental health support.
What you'll do
In this role you'll:
- Support children, young people and families to access the right mental health and wellbeing support through a new single point of access service.
- Carry out assessments of need and risk, helping people navigate services and receive timely, appropriate support.
- Work collaboratively with partners to improve referral pathways and ensure families feel supported through their journey.
About you
We're looking for someone who:
- Has experience supporting children, young people and families in a mental health, youth work, social care and community setting.
- Understands safeguarding, risk management and early intervention approaches.
- Brings strong communication and relationship building skills, with a compassionate and person centred approach.
