Solent Mind - wellbeing assessor - coastal PCN

Solent Mind seek a wellbeing assessor to support people to manage mental health in primary care services. 

What you'll do

In this role you'll:

• Provide one to one support to help people understand their wellbeing needs and build practical coping skills.
• Carry out assessments and work collaboratively to set meaningful goals and track progress.
• Connect people to wider support, including community services and specialist care when needed.  

About you

We're looking for someone who:

• Has experience supporting people with diverse mental health or wellbeing needs. 
• Brings strong communication, teamwork and IT skills within a health or community setting. 
• Is committed to person centred, compassionate support and helping people build resilience.
