Bradford District and Craven Mind – community outreach, recovery and engagement worker (CORE)
Bradford District and Craven Mind seek a practitioner to support people with severe mental illness and recovery.
What you'll do
In this role you'll:
- Provide one to one support to people living with severe mental illness, helping them stay well and achieve their goals.
- Work closely with primary and secondary care partner to ensure people can access the right support and services.
- Complete assessments, develop person centred support plans and empower people to make positive changes in their lives.
About you
We're looking for someone who:
- Has experience supporting people with mental health needs in a health, social care or community setting.
- Can build trusting relationships and provide compassionate, non judgmental support.
- Is confident managing risk, safeguarding concerns and working collaboratively with partner organisations.
