1 x 16hrs and 1 x 24hrs per week
£11.51 per hour
The Telephone Triage Practitioner will manage calls from individuals who are experiencing a high level of mental health distress, their families and professionals.
The role will work in close partnership with the NHS Crisis Team acting as a link between individual, professional and other services by ensuring continuity of support by delivering an efficient and effective person-centred service.
The Practitioner role will navigate the individuals through defined pathways to enable delivery of the right help at the right time with the right outcome. In addition the practitioner will be expected to undertake administrative duties.
The Telephone Triage Night service is delivered in unsociable hours covering Monday – Sunday 52 weeks per year between times of 5pm and 9am. These are broken down into shift patterns of no more than 8hrs per shift.
