About the role

Active Monitoring Practitioner: The post holder will be working from within the GP Surgery, delivering low level CBT based self-guided mental wellbeing support to people referred directly by their GP. The practitioner will work with patients aged 11+ and adults over 6 sessions in a goal focused way using developed tools to improve their wellbeing.  Salary :  £19,965 per annum    Hours : 5 x 35 hours per week

Mental Health Link Worker and Active Monitoring Team Leader: The postholder will be in charge of handling half of the caseload and half of the coordination of staff, managing volunteers, drop-in, and peer groups (this ratio will heavily depend on project needs) in South Warwickshire. The postholder will be responsible for providing a high-quality, service-user-focused service to local residents who are suffering from or recovering from mental health and/or wellbeing. The postholder will also work directly with individuals experiencing mental health challenges to meet their needs in terms of social factors, wellness, and other health determinants. The Active Monitoring and Mental Health Link Worker Team Leader will have to work in our partnerships of local PCN’s, community organisations and other external support agencies.  Successful applicants will be flexible, supportive, motivational and organised.  Salary :  £23,150 per annum   Hours : 2 x 35 hours per week

Mental Health Link Worker:  The post holder will help individuals to access group support that aims to equip them with coping strategies and mechanisms for self-help. Individuals can access support at all stages of their recovery journey and will be able to benefit from a range of opportunities for peer  support.  Successful applicants will be flexible, supportive, motivational and organised. Salary :  £19,965 per annum    Hours : 2 x 35 hours per week
