About the role

This new role is part of a programme of Community Mental Health Transformation that aims to fundamentally remodel the way community mental health services operate. It will contribute to breaking down the barriers for patients moving from primary to secondary mental health services and aims to embed a more joined up approach to patient care.

Within Worcestershire there is a commitment to increase local community level provision, aligned to Primary Care Networks, and the delivery of a responsive and flexible service focusing on a population health approach with greater capacity to meet all levels of mental health need. This exciting new role will work across the Wyre Forest, Malvern or the Rurals (Great Witley, Martley and Knightwick. You will be an integral part of the PCN team working closely with other health care professionals and the wider non-clinical team.

You will work directly with individuals to support their needs relating to social factors, wellbeing and wider determinants of health through delivery of workshops and group provision. You will facilitate the sharing of preventative coping mechanisms for low level mental health conditions in both individual and group settings You will support patients at all stages of their recovery journey and enable opportunities for peer support

We are looking for someone who is an exceptional communicator with experience of working positively with individuals living with mental health problems and facing complex social and emotional challenges. You will be familiar with the benefits of a personalised approach using proven tools, to support and empower people to aid their recovery. Your positive approach and the ability to really connect and engage with people, will motivate people to reach their goals and enable you to develop excellent relationships with health professionals within the PCN and wider health teams. We are looking for talented individual who can effectively juggle multiple programmes of work and maintain a calm, focused and flexible approach to work.
