Oxfordshire Mind - Mental Health Adult Facing Practitioner (x4 posts/various locations)

About the role

The Mind Partnership require Mental Health Adult Facing Practitioners (AFP’s) to join their team.  We currently have several vacancies over different locations across the county, depending on the location of each vacancy will determine which organisation you will be contracted to work with within the partnership’s model.

The candidate will work to deliver the mental health support component for the Oxfordshire County Council’s Family Solutions Plus model. Workers will be embedded within County Council hubs/pods over the county but there is currently an element of working from home.

The AFP will engage with parent/carers who require time-limited support with their mental health and wellbeing needs.   As part of the multi-disciplinary Family Solution Plus team you will promote early engagement and aim to de-escalate families in terms of social care intervention.

The Adult Facing Practitioner will take a holistic and person-centred approach to supporting adults to identify their strengths, set goals and support motivation to improve their mental health and wellbeing through a variety of interventions.  By working alongside social care, substance misuse and domestic abuse colleagues the mental health AFP’s will be available for consultation and the mutual sharing of good practice between professionals.
