Mind in the City, Hackney and Waltham Forest – Community Connector (x2)

Mind in the City, Hackney and Waltham Forest, working alongside East London NHS Foundation Trust (ELFT) and other local voluntary sector providers, is recruiting into two new exciting Community Connector roles. 

The two Community Connectors will be working as part of local neighborhood multi-disciplinary mental health teams alongside other professionals from primary care, secondary care and the voluntary sector. 

You will be supporting people with complex mental health and social needs, developing personalised, local, holistic offers with each individual and connecting them with the most appropriate support in their neighbourhoods. The role will initially focus on providing support to people remotely and connecting people to online resources.
