Mind in the City, Hackney and Waltham Forest, working alongside East London NHS Foundation Trust (ELFT) and other local voluntary sector providers is recruiting into two exciting new Community Connector roles.
The two Community Connectors will be working as part of the local neighbourhood multi-disciplinary mental health teams alongside other professional from primary care, secondary care and the voluntary sector.
You will be supporting people with complex mental health and social needs, developing personalised, local, holistic offers with everyone and connecting them with the most appropriate support in their neighbourhoods.
