Bromley, Lewisham & Greenwich Mind are working in partnership with South London & Maudsley NHS Foundation Trust (SLaM) to deliver an innovative new Primary Care Enhanced Mental Health Service for Lewisham.
The integrated team will provide support to people with long-term mental health problems within a primary care setting, helping people develop their independence, self-management skills and achieve their recovery goals.
We are looking for a Mental Health Adviser to work alongside SLaM colleagues to deliver this service.
As part of an effective Information Advice and Guidance (IAG) team you will have the caseload management experience and skills to deliver focused, person-centred, individual support and IAG on a wide range of social issues including housing, employment and socialising; liaising appropriately with specialists and professionals. Knowledge of welfare reforms and housing legislation is essential.
