The Lewisham Dementia Support Hub is a well-established service delivered by local providers working in partnership:
Greenwich MindCare is a dementia advice and information service delivered by a small team of dementia advisors working closely with Greenwich Memory Service (Part of Oxleas NHS Foundation Trust).
Our dementia services provide comprehensive person-centred support to people with dementia and their carers, alongside training, raising awareness and community development to improve the experience of living with dementia.
With experience of working in health or social care, including in community based services, you will have good communication, assessment and support planning skills. You will be a strong networker with experience of partnership working and managing a team.
