B&NES, Swindon and Wiltshire - Safety Planning Leads

About the role

We are looking for Wellbeing Practitioner – Safety Planning Leads for a new Community Services Mental Health Framework in B&NES, Swindon and Wiltshire.

Swindon and Gloucestershire Mind, Bath Mind, Rethink Mental Illness and Alabare Christian Care and Support are a Third Sector Alliance, working across the sector together with Avon and Wiltshire Mental Health Partnership NHS Trust and Oxford Health Foundation NHS Trust to provide accessible, responsive, and inclusive mental health support across the local area, as part of the national Community Services Mental Health Framework

You will focus on identifying the persons needs and over time use the recovery model to develop self-confidence and move towards the persons identified goals. This forms part of a wider system-change and a growing network of community support operating across Primary and Secondary Care settings as part of the Community Services Mental Health Framework; walking alongside and holding ongoing relationships while a person is experiencing difficulties or distress.
