Sustainability and Transformation Plans

All local areas are required to develop a Sustainability and Transformation Plan (STP), bringing together all parts of the health economy, to show how they are planning to implement the NHS’ Five Year Forward View – which including the Five Year Forward View for Mental Health.

These plans will set out how the NHS Five Year Forward View will be implemented in your area and will ultimately have an impact of local Minds and the people you support.

To deliver these plans NHS providers, Clinical Commissioning Groups, Local Authorities, and other health care services have come together to form 44 STP ‘footprints’. The size of each footprint varies from covering 300,000 people to 3 million. The map below shows the footprints, our network of local Minds and CCG areas.

Listed within it are the names of the person who is leading on the development of your local STP, as well as a link to a summary document of your local plan, where it is available
