I give permission for Mind to use my video in the Mind website and other Mind sites including social media (e.g. Youtube, Facebook, Twitter etc.) as part of their work to support people with mental health problems

I am happy that they can share it via email social media (such as Facebook and Twitter) and in emails to their supporters.

I am happy that they may use my mental health story in communications with potential or actual partners and supporters (e.g. internal and external training, possible donors, corporate partnerships etc.)

I agree that I am happy for them to edit parts of my 'mental health story' and that they may go alongside other professional or volunteer's videos
- This vlog is mine, I filmed it myself and anyone who appears has agreed to feature
We (Mind) agree that:

We will host your video on our Youtube channel and moderate and manage comments on your behalf
- We will not use your information for anything other than to contact you about the mental health story project (See our privacy policy)
