Tameside, Oldham and Glossop Mind – living well navigator

In your role as Living Well Navigator, you'll work as part of the Living Well huddle within your allocated Primary Care Network (PCN), and with partners across the borough to facilitate wraparound care for individuals. Navigators play a key role in the Living Well model, supporting people to overcome barriers in accessing services. 

What you'll do

In this role you'll:

  • Communicate effectively, verbally and in writing, to a wide range of people inside and outside the organisation
  • Use IT systems including email, online systems and Microsoft packages to record and share information
  • Deliver and monitor a range of complex activities or programmes in your department
  • Achieve service and project outcomes
  • Work in partnership with a wide range of partners, stakeholders, and communities
  • Support the organisation to devise and develop improvements in your area of expertise
  • Carry out any other reasonable tasks relevant to the role

About you

We're looking for someone with:

  • Experience of providing support services to members of the public
  • Experience of working independently to deliver services safely
  • Willingness to undertake training and qualifications relevant to role
  • Prior qualifications in mental health interventions, for example counselling skills or coaching
  • Knowledge of emotional wellbeing, mental health issues or other relevant health
  • Knowledge of approaches to support people to achieve better mental health
