South Warwickshire and Worcestershire Mind – mental health practitioner

Active Monitoring is a psycho-educational programme designed to provide early intervention within the Primary Care arena. This role will involve working directly from a GP surgery delivering the Active Monitoring programme.

What you'll do

In this role you'll:

  • Be responsible for a caseload of Active Monitoring clients
  • Work with people to overcome early signs of mental health distress such as anxiety, depression and stress
  • Deliver the service and where necessary guide/actively refer to other services
  • Follow a set programme, including referring people to higher levels of intervention when required
  • Manage all administration processes accurately and efficiently

About you

We're looking for someone with:

  • 12 months’ experience using social care and/or coaching skills to work face to face with the public, some of whom may present as emotionally upset or angry
  • Level 3 qualification in health and social care or equivalent
  • 6 months' experience providing mental health/wellbeing services, preferably within a primary care setting
