About the role

The community connector will work in the neighbourhood team in Stepney and Wapping primary care network.

You'll make sure people with serious mental illnesses are able to live fulfilled lives.

You'll do this by offering one-to-one support to help people set goals towards their aspirations.

The community connector is seen as valuable asset to our neighbourhoods and acts a local expert, gathering and sharing information about local services.

You'll work with other teams to create pathways of support for people. You'll act as the ‘glue’ linking people to experts and local services. 

You'll support people to identify the wider issues that impact their health and wellbeing, like debt, poor housing, unemployment, loneliness, isolation and caring responsibilities.
