Mind in Brent, Wandsworth and Westminster – community programmes coordinator

In this role you'll coordinate care for service users through the Active Wellbeing programme, meeting service objectives and the requirements of service level agreements and contracts.

What you'll do

In this role you'll:

  • Be an active member of the Mind in Brent, Wandsworth and Westminster Community Programmes team, working with colleagues to coordinate and deliver activities through the Active Wellbeing programme, as agreed with line manager
  • Identify, assess and plan support with service users, undertaking risk assessments and monitoring as required
  • Handle eligibility queries
  • Liaise with referrers as part of referral pathway
  • Lead on correspondence and decision-making related to current mental health or physical health with clients
  • Lead on signposting, suitability of re-referrals, identifying motivations and exploring alternatives, e.g. drop-in gym

About you

We're looking for someone with:

  • Education to degree level or equivalent in a relevant or related field
  • Experience of working within a social care setting and/or supporting people with mental health problems
  • Experience of delivering person-centred services
  • Experience of recruiting and supervising volunteers and/or staff
  • Experience of training groups and delivering presentations
  • Experience of project monitoring and quality assurance
  • Good understanding of how to work within a fixed budget and to deadlines
  • Good communication skills, both verbally and in writing, with an ability to listen actively
  • Intermediate MS Office skills (including Word, Excel and PowerPoint) with an ability to use database systems effectively
  • Initiative to think ahead and take prompt action to solve problems and seize opportunities
