Mind in Bradford – wellbeing practitioner (SMILE)
This role is required to deliver high-quality mental health care and support. Our work goes beyond mental health to address various issues impacting individuals' wellbeing, such as employment, welfare and benefits, social housing, money management, social isolation, and bereavement.
What you'll do
In this role you'll:
- Offer personalised one-to-one support, over the phone or in person, to clients from diverse backgrounds dealing with various mental health challenges
- Possess in-depth knowledge of the support services available throughout the district
- Skilfully direct clients to relevant resources and services to amplify their support network
- Offer support around various issues impacting individuals' wellbeing, such as employment, welfare and benefits, social housing, money management, social isolation, and bereavement
- Conduct individual assessments and leverage outcome measures to create customised support plans, enabling individuals to proactively manage their wellbeing
About you
We're looking for someone with:
- Experience of working with adults in a health, social care or other relevant setting
- Good knowledge of services across the Bradford District & Craven
- Excellent communication skills and feel confidence in supporting people in a clear, meaningful and non-judgemental way
- Confidence in recognising and responding to risk and safeguarding concerns
- Ability to take initiative and be proactive in solutions
- Good organisational, financial and IT skills
