Mind in Bradford – wellbeing practitioner

The wellbeing practitioner will support clients face to face to enable them, their families and carers to access personalised care and support during the process of being discharged from hospitals or with a need in the community. 

What you'll do

In this role you'll:

  • Support service users from a variety of backgrounds
  • Provide support with finance-related tasks including making purchases for clients
  • Maintain a record of all transactions made, including the recording and uploading of all evidence
  • Maintain electronic records of support, including client information
  • Motivate, lead and encourage client participation at all levels to support them to take ownership and manage their own mental health

About you

We're looking for someone with:

  • Experience of working with adults in a health, social care or other relevant setting
  • Good knowledge of services across the Bradford District & Craven
  • Excellent communication skills and confidence in supporting people in a clear, meaningful and non-judgemental way
  • Confidence in recognising and responding to risk and safeguarding concerns
  • Ability to take initiative and be proactive in solutions
  • Good organisational, financial and IT skills
