Dorset Mind – active monitoring practitioner (Purbeck)
We have an exciting opportunity in the Adult Services team at Dorset Mind. We are looking for an Active Monitoring Practitioner to work for Purbeck Primary Care Network and deliver early intervention for people over the age of 18 experiencing mild to moderate mental health problems.
What you'll do
In this role you'll:
- Raise awareness within communities about mental ill health
- Educate people about symptoms
- Challenge the stigma and inequalities of mental ill health
- Promote the ethos of recovery from mental ill health
- Promote positive wellbeing and early intervention methodologies
- Help with networking and promoting Active Monitoring and other Dorset Mind services
About you
We're looking for someone with:
- Relevant qualification for the role, for example NVQ Level 3 in Health & Social Care
- Familiarity with the vision and mission of Dorset Mind and the Mind network
- Knowledge of the issues of supporting people with a range of mental health issues, including risk assessment and mental health recovery
- Ability to deliver the service as a standalone intervention and provide psycho-educational skills to enhance wellbeing
- Ability to offer various pathway options and promote client autonomy
- Ability to identify and manage risk incidents
- Ability to assess clients for suitability and make appropriate referrals
- Good IT skills and ability to carry out a range of office tasks
